Multiple Controls Required to Deal with Human Errors

A great contribution to the field of human error prevention is James Reason’s swiss cheese model.  It emphases that different levels of controls are necessary in order to prevent human errors.  Here’s an example of a situation where there were multiple controls, but one failed and the other wasn’t used, causing a significant surgical error.
